Why Uroguidance?


As CMS shifts towards reimbursement models which encompass care both during and immediately after a patient’s stay in the hospital, referring hospital systems are increasingly focused on both the quality and value of care provided by nursing facilities to their patients after hospital discharge. To effectively address the challenges of improving both the quality and value of care being provided in nursing facilities, a more integrated cooperative approach between these facilities, the hospital systems, and specialty care providers is needed. The goal of the Uroguidance program is to help fill the unmet need of providing specialty urologic care to the nursing facility population. Better urologic care provided directly to the residents has the potential to benefit everyone involved. Residents of the facility receive optimized urologic care without having to leave the facility. The nursing facility receives specialized urologic support for their patients as well as assistance with meeting state inspection requirements in regards to urinary health areas of evaluation at no extra cost. Improved urologic care after discharge also helps local referring hospital systems by reducing global costs associated with poor outcomes, such as unplanned hospital readmissions.
