1) Credentialling process for Uroguidance providers
2) Introduction letter to primary care providers explaining the program
3) Education
a) Nursing staff: Powerpoint
b) Medical staff: Powerpoint, UTI article, Urofacts
4) Identification of key personnel
a) Admissions person for Consent form (and where list will be placed)
b) Person who has list of urine cultures from past week (and where list will be placed)
c) MDS 672 Report- to identify long-term residents with urinary incontinence and UTIs
d) Outcomes data person
5) Contact information for:
a) Administrator
b) Director of Nursing
c) Emails of medical care providers (for UroFacts)
